Almoner

Almoner is a system design practice for health and human services. We build data infrastructure and implement products that facilitate housing, health and social care programs working together: shared data models, interoperability standards, and technical assistance that funders and front-line staff can both trust.

The name comes from the hospital almoner, who from the 1890s assessed patients’ circumstances and connected them to help beyond the ward, situating the early history of medical social work. The problem remains the same today: care that depends on information and collaborative work across institutional boundaries.

We specialize in resolving fragmentation across the ways that housing, health, and social services coordinate care and exchange data without replacing every source system.
